01Acceptance of Terms

By booking an appointment, visiting our salon, using our website at herbalnailsarrowheadlakes.com or opting in to our SMS program, you agree to these Terms of Service ("Terms"). If you do not agree, please do not use our services.

These Terms form a legal agreement between you and HENNA BY PURVI LLC, an Arizona limited liability company doing business as Herbal Nails & Spa at Arrowhead Lakes ("we," "us," "our" or "the salon").

02Services Provided

We offer nail care, spa, beauty and wellness services including but not limited to manicures, pedicures, nail enhancements, eyelash extensions, facials, waxing and permanent makeup. Service descriptions, durations and prices are listed on our website and in-salon menu and are subject to change without notice.

We reserve the right to refuse service to anyone for reasons including (but not limited to) safety, hygiene, behavior or capacity.

03Appointments & Booking

Appointments may be booked online, by phone or in person. Walk-ins are welcome and accommodated based on availability. To confirm your booking, we may require a phone number, email or in some cases a deposit.

Please arrive on time. Arriving more than 10 minutes late may result in your service being shortened or rescheduled at our discretion to keep other guests on time.

04Cancellation & No-Shows

We respect your time and ask the same in return. To cancel or reschedule, please give us at least 24 hours' notice.

• Cancellations made with less than 24 hours' notice may incur a fee of up to 50% of the service price.
• No-shows may be charged 100% of the booked service price and may be required to prepay for future bookings.
• Groups, parties and longer appointments (over 60 minutes) may require 48 hours' notice.

05Payment & Refunds

Payment is due at the time of service. We accept cash, major credit cards and approved digital payment methods. All prices are in U.S. dollars and exclude any applicable taxes.

Refund policy: Service fees are generally non-refundable once a service has been completed. If you are not satisfied with a service, please let us know within 48 hours of your visit and we'll make a reasonable effort to correct the issue. Product purchases are non-refundable if opened or used.

06Gift Cards & Memberships

Gift cards are non-refundable, non-transferable and may not be redeemed for cash (except where required by law). Lost or stolen gift cards cannot be replaced.

Memberships (such as our lash membership) have specific terms communicated at the time of enrollment, including the non-refundable annual membership fee. Member benefits are not transferable.

07Salon Conduct

To maintain a relaxing environment for all guests, we ask that you:

• Treat our staff and other clients with respect and kindness
• Keep phone conversations brief and at a low volume
• Supervise children who are not receiving services
• Refrain from intoxication or disruptive behavior

We reserve the right to refuse service or ask anyone to leave who violates these expectations.

08Health, Safety & Liability

Please inform your technician of any allergies, sensitivities, skin conditions, medical conditions, recent surgeries or pregnancy before your service begins. Some services may not be suitable in certain conditions.

While we take every precaution, you understand that some services involve inherent risks (e.g. minor irritation, allergic reactions). By receiving services, you acknowledge these risks and agree that Herbal Nails & Spa at Arrowhead Lakes, HENNA BY PURVI LLC and our staff are not liable for any minor reactions, damages or losses arising from the proper performance of services, except where caused by our gross negligence or willful misconduct.

You are responsible for any personal belongings you bring into the salon. We are not responsible for lost or stolen items.

Reply STOP to any message to unsubscribe at any time. You will receive a final confirmation message.

Eligibility

You must be 18 years or older and the authorized account holder of the mobile phone number you provide.

Privacy

We do not sell, rent or share your SMS opt-in data or mobile phone number with third parties for marketing. See our Privacy Policy for full details.

Carriers

Supported carriers include T-Mobile, Verizon, AT&T, Sprint and other major U.S. carriers. Carriers are not liable for delayed or undelivered messages.

10Website Use

You agree to use our website lawfully and in accordance with these Terms. You will not:

• Use the site for any illegal purpose or in any way that could damage or disable it
• Attempt to gain unauthorized access to any part of the site or our systems
• Copy, scrape or republish content from the site without our written permission
• Upload viruses, malware or any harmful code
• Submit false information when booking or contacting us

11Intellectual Property

All content on our website and in our salon (including the Herbal Nails & Spa name, logo, design elements, photos, videos and written content) is the property of HENNA BY PURVI LLC or its licensors and is protected by U.S. copyright, trademark and other intellectual property laws.

You may not use, reproduce or distribute any of our content without our prior written permission.

12Disclaimers & Limitations

Our services and website are provided "as is" and "as available" without warranties of any kind, either express or implied, to the fullest extent permitted by law.

To the maximum extent allowed by law, in no event shall Herbal Nails & Spa at Arrowhead Lakes, HENNA BY PURVI LLC or our owners, employees and affiliates be liable for any indirect, incidental, special, consequential or punitive damages arising out of or in connection with your use of our services or website.

Our total liability to you for any claim arising under these Terms is limited to the amount you paid us in the six months preceding the claim.

13Governing Law

These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-law principles. Any disputes arising from these Terms or your use of our services shall be resolved in the state or federal courts located in Maricopa County, Arizona. You agree to submit to the personal jurisdiction of these courts.

14Changes to Terms

We may update these Terms at any time. The most current version will always be posted on this page with an updated "Last Updated" date. Your continued use of our services after changes are posted constitutes your acceptance of the updated Terms.
